Chronic Disease Self Management Program

Classes are provided both in English and Spanish and are entirely FREE for those who wish to attend. Below you will find an explanation of:

Tomando Control de su Salud is a workshop given two and a half hours, once a week, for six weeks, in community settings such as senior centers, churches, libraries and hospitals. Spanish-speaking people with different chronic health problems attend together. Workshops are facilitated by two trained leaders, one or both of whom are non-health professionals with a chronic diseases themselves. All workshops are given in Spanish or English without translators.

The Program is not a translation of the Chronic Disease Self-Management Program, but developed separately in Spanish. Subjects covered are similar, but they are presented in ways that are culturally appropriate. Subjects include:

  1. Techniques to deal with problems such as frustration, fatigue, pain and isolation
  2. Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  3. Appropriate use of medications
  4. Communicating effectively with family, friends, and health professionals
  5. Healthy eating
  6. Appropriate use of the health care system
  7. How to evaluate new treatments

Each participant in the workshop receives a copy of the companion book, Tomando Control de su Salud: Una guía para el manejo de las enfermedades del corazón, diabetes, asma, bronquitis, enfisema y otros problemas crónicos. All materials are in Spanish. For the English course the book participants will receive is Living a Healthy Life with Chronic Conditions.

This book is filled with hundreds of proven tips, helpful suggestions, and concrete strategies to deal with chronic illness, to manage emotions, and to live a productive life. Here you'll find suggestions for setting goals, making decisions, and finding resources and support. It offers sound information about exercise, healthy eating, intimacy and sex, and communication with friends, family, and caregivers. There is also information on managing medications and an important new chapter on making treatment decisions.

Originally based on a 5-year study conducted at Stanford University, this work has grown to include the feedback of medical professionals and people with chronic conditions all over the world. Living a Healthy Life with Chronic Conditions has helped thousands of people with chronic illness fulfill their greatest possible physical potential and to once again derive pleasure from life. It is the process in which the program is taught that makes it effective. Classes are highly participative, where mutual support and success build the participants' confidence in their ability to manage their health and maintain active and fulfilling lives.

Tomando Control de su Diabetes is a workshop given two and a half hours, once a week, for six weeks, in community settings such as senior centers, churches, libraries and hospitals. Workshops are facilitated by two trained leaders, one or both of whom are non-health professionals with diabetes themselves. All workshops are given in Spanish without translators.

Spanish-speaking people with diabetes attend the program in groups of 10-16. Workshops are facilitated from a highly detailed manual by two trained leaders, one or both of whom are peer leaders with diabetes themselves. Participants may also bring a family member or friend.

Subjects covered include: 1) techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration; 2) appropriate exercise for maintaining and improving strength and endurance; 3) healthy eating 4) appropriate use of medication; and 5) working more effectively with health care providers. Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Physicians and other health professionals both at Stanford and in the community have reviewed all materials in the workshop.
